Tellurian UCAN, established in 1975 and based in Monona, Wis., provides treatment to patients suffering from substance abuse. Each patient's treatment plan is based upon a full assessment from which individual needs are identified. The institution also serves patients considered dually diagnosed with mental health diagnoses in addition to their substance abuse diagnosis. Tellurian UCAN provides adolescents residing in Dane County of Wisconsin with intensive outpatient services funded by the Dane County Department of Human Services. The network program provides screening and assessment services and develops treatment plans based upon the results. It also provides services to the families of patients. The institution additionally provides a range of services for individuals with severe and persistent mental illness and in need of supportive housing within the community. It also provides shelter programs for homeless communities.
