Founded in 1978, Hospice of the Western Reserve provides palliative care, caregiver support and bereavement services. The center offers various levels of services to meet the needs of patients, families, friends and caregivers. It provides services in homes, hospitals, hospice houses, and assisted living and long-term care facilities. Located in Mentor, Ohio, the center offers pain management and symptom control plans and 24-hour continuous care to manage pain or acute medical symptoms. It additionally provides respite, hospice and inpatient care services. The center also offers specialized programs on heart, lungs, AIDS and pediatric care. It is a Medicaid- and Medicare-certified center that is a member of the National Hospice and Palliative Care Organization and the Ohio Hospice and Palliative Care Organization.
