Established in 1974, Urban Health Plan is a nonprofit community health center that offers diagnostic treatment and health care programs and services. It offers a selection of primary and ancillary care services, including internal medicine, pediatrics, adolescent clinic and family medicine. The agency specializes in audiology, surgery, pulmonology, physical medicine, physical therapy, neurology, cardiology, psychiatry and radiology. Its facilities include main site, satellites, more than four school programs, part-time facilities, and administrative program sites that house public health and other grant-funded programs. It has alliances with several community-based organizations that provide health care services to local schools, adult day treatment programs and homeless shelters.
