Finger Lakes Performing Provider System (FLPPS) is a network of clinical and community-based provider organizations in the Finger Lakes region of New York. Their mission is to transform the healthcare delivery system by bridging the gap between the community and healthcare. They have successfully implemented the New York State Delivery System Reform Incentive Payment Program (DSRIP) and continue to drive transformation through strategic pillars, including community programs, supporting partners, and workforce development.
FLPPS focuses on improving population health, reducing avoidable hospitalizations, and lowering healthcare costs. They have achieved significant milestones, engaging thousands of patients and earning eligible dollars for their partners. In addition to their DSRIP programs, FLPPS invests in innovative efforts to address social, racial, and health inequity gaps, particularly exacerbated by the COVID-19 pandemic. They collaborate with community organizations, stakeholders, and their subsidiary organization, Greater Rochester Health Home Network (GRHHN), to provide care management, care coordination, and address social determinants of health.
