The Brockton Visiting Nurse Association was established in 1904. It is based in Brockton Mass., and is a subsidiary of Bay State VNA and Hospice. It serves Plymouth, Bristol and Norfolk counties in Massachusetts. The association is one of the largest visiting nurse associations in the commonwealth, and employs more than 200 health care professionals who provide services to people of all ages. The association offers a wide variety of services, such as nursing, maternal child health; rehabilitation therapy for adult and pediatric; medical social services; registered dietician and home health aides. Its services also include disease management, patient education, pediatric nursing, home phototherapy, ultrasound, physical and occupational therapy, speech and language pathology, equipment evaluation and case management. Brockton Visiting Nurse Association is accredited by Joint Commission on Accreditation of Healthcare Organizations.
