History
When originally built it was Rock Falls Colonial Acres a 55 bed nursing facility. It was built to fill a growing need of caring for the chronically ill and elderly. Colonial Acres became Transitions in 2007 celebrating the 40th anniversary. Since then the rehabilitation department has been expanded and private rehab suites were developed. In doing this the capacity was decreased. This in turn makes us a very personalized and more home like atmosphere.
Specialties
We specialize in Short term rehabilitation with PT OT & Speech therapy, and have private rooms available. We can accommodate long term & respite stays. We work with many different Hospice companies making the patients preference 1st priority. The staff here are very compassionate and caring. They not only become part of the patients life, but the family as well. Being a small facility cultivates the personalized attention each person deserves.
