Primary Care of of Southwest GA, Inc was established in 2005 and opened a Federally Qualified Community Health Center for business in Blakely, Georgia, in March 2006. In August 2007, a second site was opened in Thomasville and was approved as a Federally Qualified Health Center Look Alike in September 2008. PCSG is a non-profit corporation which is governed by a local board of directors made up of community members, 51% of whom are required to be active patients. PCSG serves the broad health needs of Southwest Georgia residents through a family practice model of care. PCSG works to ensure that its patients have access to all levels of the health care system, and is especially committed to providing services for the medically underserved. PCSG is a strong advocate of preventive education and public health activities. Primary Care of Southwest Georgia collaborates with other community agencies and organizations, i.e. Local Hospitals, Spring Creek Health Cooperative, and Public Health, to identify health related needs and to develop resources to facilitate these needs. Primary Care of of Southwest GA, Inc. is a general practice community health center, which means that our providers treat most common illnesses, injuries, and conditions. However, you or a member of your family may need care from a physician who specializes in a particular area of medicine. In this case, a referral will be made. This center is part of a referral system involving several local and regional medical centers, hospitals, and physicians.
