History
Established in 1968. We are the first community health center founded in Connecticut. Established in 1968 in a collaboration between the community and Yale School of Medicine. The first community health center in Connecticut, the Cornell Scott-Hill Health Center has a long history of serving New Haven neighborhoods, which are among the most disadvantaged in the State. Cornell Scott-Hill Health Center also provides health care services to those from the City of West Haven and towns in the Lower Naugatuck Valley, Ansonia, Derby, Seymour, Shelton, Naugatuck, and Oxford.

The Cornell Scott-Hill Health Center is proud to be a Patient-Centered Medical Home (PCMH). A PCMH is a system of care that creates a strong partnership between you and your personalized care team. Your team will be led by your primary care provider and may include nurses, health educators, specialists or any other medical personnel who can help you achieve your health and wellness goals. As partners committed to you and your wellness, we'll work with you and your family to coordinate easy access to care. We'll help you select a primary care provider and will coordinate your care using our electronic health records. We'll involve you in every step of your care plan and teach you how to self-manage chronic conditions in order to stay healthy
