Established in 1952, Visiting Nurse Association and Hospice of Southern California is a nonprofit, community-based health care agency serving people of all ages. It provides various case management services, such as development and implementation of the plan of care, patient advocacy, and caregiver training and supervision. It additionally offers skilled nursing intervention services, including physical assessments, disease process monitoring, medication assessment and instruction, patient education, nursing procedures, maternal postpartum care, and medication assessment and instruction. Its enterostomal therapy services include ostomy management and instruction, preoperative site marking, wound drainage containment, fistula management, and preoperative and postoperative patient education. In addition, the center offers a palliative care program, infusion therapy, and rehabilitation and mental health services.
